Applications
can be made in person at the IEP office or sent by mail, fax, e-mail
or Internet (an original financial statement must be mailed).

Completed Application for Enrollment / I-20 form.
Application fee is(non refundable. Please check Other Fees page for details.
Please use cashiers check/money order made payable to UC Regents or credit
card.
IMPORTANT NOTE
Credit card information is only accepted via FAX, telephone, or surface
mail. If you are downloading, printing and sending your application
via surface mail or FAX, you may enter your credit card information
on the form. If you are downloading and completing your file on the
computer and sending to IEP as an e-mail attachment, do NOT enter any
credit card information on the form. Credit card charges under $100
will be accepted over the telephone. Credit Card charges over $100
will be accepted via FAX and surface mail.

A non refundable deposit of $100, made payable to UC Regents.
This will be applied to your tuition upon enrollment.
